Angel Alert

Please answer all of the following questions:

Upon approval of this application, your Angel Alert Device will be shipped to the address you have provided to us, within 5 business days.  You will not be charged for shipping or for any equipment rental.


Included with your device will be instructions on how to set up the device.  You will need to download the POM System APP and to follow the directions.   If you have any questions or if you should need any assistance, contact POM Safe directly at 732-687-9767.  You will be responsible for setting up the device and for testing it with the Alert Center to ensure that it is in good working condition and to confirm that the equipment was received undamaged.


If the device does not work or malfunction due to an equipment defect or if was damaged during delivery, you must contact POM Safe at 732 687-9767 immediately to resolve the issue.


PROGRAM AGREEMENT 


Your Usage Agreement will remain in force for (6) six months from the Start Date listed on this agreement. Approximately two weeks prior to the expiration date, an SNPF representative will contact you to review your status and determine whether or not to extend the Agreement for an additional (6) six months. 


If it is determined that service is no longer approved or necessary, the SNPF representative will terminate the Usage Agreement and service will end.      **   (See responsibility section below)

READ CAREFULLY!

You are responsible for the following: 

  • The initial set up of the device with POM Safe Call Center upon receiving the equipment, in order to activate the service.
  • Maintaining and charging the equipment so that it remains in good working condition at all times.
  • Reporting any damage to the equipment that may occur, rendering the device inoperable.  
  • Lost or stolen equipment.   (Report Immediately)!
  • **    Equipment that is not returned within 10 business days of the Agreements Termination date.
  • Ensuring that the device is connected to a Blue Tooth source.
  • To update your profile information whenever it changes with the call center via the POM App.

A $70.00 fee will be charged for any Lost, Stolen, (non-repairable) Damaged Equipment or  

** Equipment that is not returned upon completion of agreement.


If the Usage Agreement is extended for an additional six months, two weeks prior to the extended Usage Agreement’s expiration date, a representative from the SNPF will contact you to offer you options for continuing the service or to end the service entirely.  


If you decide NOT to continue with the program for any reason, all equipment must be returned to the SNPF, undamaged, and within 10 business days.  Failure to do so will result in you being held liable for any monthly service fees, late fees, plus the replacement cost of the equipment $70.00.  

If you are not approved for an extension of use, all equipment must be returned to the SNPF. Failure to do so will result in you being held liable for any monthly service fees, late fees, etc. Plus the replacement cost of the equipment $70.00. 

If any of the equipment has been damaged beyond repair, upon receipt by the SNPF or POM Safe, You will be held responsible for the replacement costs of $70.00.

The SNPF is not responsible for and may not be held liable for, any of the services or equipment POM Safe offers, provides or is responsible for.  


The SNPF’s sole responsibility is to ensure, that the client receives the equipment at no cost to them, for up to 1 year, as per the usage agreement.  


Under no circumstances shall the SNPF be held liable or responsible for any type of equipment failure or malfunctions or for anything as a result of such failure or malfunction.


The SNPF is not responsible for any installation or instruction for use of any of the POM Safe devices or equipment.  Nor are they responsible for any monitoring or emergency dispatch services.
